Notes on Failure Attribution

 

 

Failures in complex systems rarely originate where they are reported.
They emerge at the intersection of delayed feedback, constrained adaptation, and misaligned accountability.

When systems are unable to observe themselves at the scale where consequences form, failure is displaced downward and reinterpreted as individual error.
This displacement stabilizes the system while obscuring its structural behavior.

High-reliability technical systems treat failure as a design signal.
Many social and organizational systems treat failure as deviation to be corrected.

As feedback latency increases, attribution shifts from architecture to actors.
Correction follows blame rather than redesign.

Over time, systems that cannot surface their own failure modes preserve coherence by normalizing loss.
Stability is maintained, not by resilience, but by tolerance for irreversible error.
